Remediation tracking: no closure before risk changes

Follow-up separates impact correction, implementation and effectiveness. Plans carry outcome, owner, due date, evidence and criterion, and status changes through evidence. Implementation proves deployment, effectiveness tests post-date population and lookback covers history. Extensions and risk acceptance require authority; closure is a memo with residual risk and reopen trigger.
